How a Special Needs Plan works

A Special Needs Plan (SNP) is a type of Medicare Advantage plan that combines all the benefits of Original Medicare (Parts A and B) with prescription drug coverage (Part D). SNPs are only available to people who have an additional qualifying condition or are eligible for both Medicare and Medicaid.

Types of Special Needs Plans

Chronic Condition Special Needs Plans (C-SNPs) offer special care for people with diabetes, cardiovascular disorders, chronic heart failure, chronic lung disorders or other qualifying conditions.

Dual-Eligible Special Needs Plans (D-SNPs) are for people who have both Medicare and Medicaid. With a D-SNP, all your Medicare and Medicaid benefits are combined into 1 easy-to-manage plan.
