  • Name and policy information for any other insurance coverage you might have
Long-term care information
  • If you are a current resident of a long-term care facility, address of facility,
  • If you are moving into or out of a long-term care facility, date of your move
Contact information for your healthcare professionals
  • A list of your doctors and other healthcare professionals
Prescription drug information
  • Name and address of your pharmacy
  • List of your current prescription drugs and dosages
Personal health information
  • Current health conditions
  • Any ongoing treatments you are currently receiving (e.g., chemotherapy, dialysis, etc.)
Plan premium payment information
  • If you enroll in a plan, you will be asked how you’d like to pay your plan premium; common options include automatic withdrawal from your monthly Social Security check or electronic funds transfer (EFT) from a bank account
