A plan is cancelled when you request to leave the plan before the plan’s effective date.
How to Cancel

Note: Calling or faxing the request is preferable if the plan’s effective date is soon.
Include this information in the written request:
Member’s ID number
Plan name, such as Humana Gold
Statement of intent to cancel
Note: If you pay via EFT or credit card, you are responsible for canceling your plan at least five days before the next billing cycle begins. If you pay by coupon, simply stop sending payment. The application fee is non-refundable.
If you are currently enrolled in a Medicare Advantage plan that doesn’t include dental, you can add a dental or vision plan on anytime during your coverage year. Go to Additional Coverage section https://www.humana.com/medicare/products-and-services/supplemental-benefits/ and review your options, then contact a Humana licensed sales representative at 1-800-457-4708 TTY Users: 711 Hours Monday- Friday 8 a.m. – 8 p.m.
Sign in or Register for MyHumana and go to the eBilling link on the left hand side. You can only make your initial payment at that time, but after that first payment has cleared your financial institution, you can then set up recurring payments.
You'll need your member ID number, date of birth and ZIP code in order to register
Usually your provider submits your claims. If you have a claim that wasn't submitted by a provider, download the claim form from MyHumana's Member Support. Follow the instructions on the form to submit your claim.
