* The Humana Premier Rx Plan (PDP) provides affordable, predictable copayments on insulin. Members participating in the Insulin Savings Program may pay even lower copays for Select Insulins and the copay is the same for both vials and pens. The Insulin Savings Program is part of the Part D Senior Savings Model from the Centers for Medicare and Medicaid Services (CMS). Select Insulins include all of the preferred insulins on Humana’s formulary, including rapid-acting, short-acting, intermediate-acting, and long-acting insulin vials and pens.

90-day supplies are available for most drugs on Tiers 1–4 (subject to quantity limits).

Be sure to check the Drug Guide to see which insulins are covered. You can also find your other medications’ tiers and choose a plan based on those costs. You’re not required to have a prescription drug plan, but you may face a late enrollment penalty for not enrolling when you’re first eligible for Medicare.
