Living with diabetes

If you have diabetes—or are at risk for developing it—sign in to MyHumana to see what you can do next, keep tabs on your blood work or other labs and get information on your benefits.

Focused support for chronic conditions

Members living with ongoing health conditions may be eligible for additional assistance from a nurse, case manager or social worker.

Care management

Our care managers work with eligible members one-on-one to help them achieve their best health while living with a chronic condition. The care manager will offer support by listening to your needs and checking in regularly by phone, video or home visit.

Care for complex health needs

Eligible members living with multiple chronic conditions may receive additional personalized support over the phone. This could include care plan development and oversight, referrals for services and help reaching health goals.

Help with advanced care

When you need to make sensitive healthcare decisions, we’re here to help. That could mean palliative or hospice care or connecting you with tools for telling doctors or family how you want your care to be handled.
