Changes to your contact information

Humana asks participating physicians, hospitals, facilities and other healthcare providers to promptly update their contact information (service address, phone/fax numbers, etc.) when changes occur. There are three ways to make these changes:

  1. Contact your regional contracting representative, PDF opens new window. (Please use Humana’s preferred standard roster template, PDF opens new window or a similar template format to submit your demographic information to your contracting representative.)
  2. Use Humana’s demographic updates form to submit your changes.
  3. Update information through Availity Portal, opens new window.

Behavioral health professionals
For Humana behavioral health service preauthorization requests and notification, please call 800-523-0023. This line is open 24 hours a day.

Please promptly update your service address and other contact information (address, phone/fax numbers, etc.) when changes occur. To do so, you may send an email to behavioralhealthproviderservices@humana.com with your updated contact information. Updates also can be made through Availity Portal.
