How to update your information

Contact your regional representative by using our regional contact map below. Send an email to your representative with the subject line: Demographic Updates.

  • If you’re submitting updates for a large group please email your information and use our preferred standard roster template or a similar template format.

  • If you are adding a location or making an update to existing information for a single provider you may use the demographic updates form.
