Effective April 1, 2015, Humana will require prior authorization for Diovan for most Medicare formularies. Prescribers with patients currently taking Diovan may want to consider switching them to valsartan, its generic equivalent, if clinically appropriate. Valsartan is available on impacted formularies in the same preferred brand tier as Diovan and will not require prior authorization.

Prescribers may use the Provider Drug List Search tool to determine medications covered by their patients’ Humana health plan. The search will display the coverage level of the drug on specific drug plans, as well as any coverage limitations (e.g., quantity limits, prior authorization requirements, step therapy, etc.).
