Due to a system error, some Humana health care providers received invalid rejections for institutional claims submitted with a Medicare bill type of 11x. Affected claims were submitted between June 28, 2016, and July 8, 2016.
The affected claims contain patient status codes of 02, 03, 05, 50, 51, 61, 62, 63, 65, 66 or 70 and were not billed with condition code 40. Claims with the same admit and discharge dates were not affected.
The message received is: Error Message: Category: Acknowledgement/Rejected for Invalid Information The Claim/Encounter has invalid information as specified in the Status details and has been rejected Status: NUBC Condition Code(s)
Humana has corrected the technical issue and will reprocess impacted claims. No action is required on the part of health care providers.
Professional and dental claims and claims submitted via paper are not affected.
