How patients get access

Typically, a discharge nurse or a Humana care manager will enroll a patient. But, Humana-covered patients can call Humana Customer Care at 800-457-4708 (TTY: 711) to see if they are eligible.

Who is eligible

Most Humana Medicare Advantage group and individual plans as well as some Medicare Supplement and Humana chronic-condition special needs plans.

About the meals

Our meals are provided by Humana vendors and are made with high-quality ingredients, designed by dietitians, so patients can receive meals specific to their health, chronic condition, or post-discharge recovery process. All meals arrive ready to heat-and-eat and last up to 14 days in the fridge.
