In partnership with the Louisiana Department of Health (LDH), we will work with our provider network to serve our members in Louisiana and help them reach their health goals and get the right care in the right place at the right time.

All providers are required to have an active Medicaid ID number. If you do not have a Medicaid ID number, please visit the Louisiana Department of Health’s application portal.

LDH sent all current providers a letter explaining the process for enrollment with instructions. Providers will need several data elements from this letter to complete their enrollment, including their Louisiana provider ID, National Provider Identifier (NPI), city, state, and ZIP code.

A provider who does not have this letter or this information can email louisianaprovenroll@gainwelltechnologies.com to request a reprinted letter be mailed. Email requests must include the provider name and the NPI. Providers may send multiple provider requests in a single email.

Reprinted letter requests will only be accepted by email. No other form of submission is accepted. Providers will receive a confirmation email from Gainwell Technologies when the submission is received. That email will include an anticipated turnaround time for the response.

After you apply

After you submit your online application, we will:

  • Review your application and contact you for additional or clarifying information
  • Contact you to initiate a participating agreement
  • Ask you to:
    • Review the participating agreement
    • Sign and return it to us

Credentialing may be required before an agreement becomes effective. We may ask you to submit additional information to complete credentialing. Once credentialing is complete, you will:

  • Get a copy of the contract, which will be signed by a Humana representative
  • Learn your effective date with Humana

Our contracted providers get:

  • Ongoing support and learning opportunities
  • Timely and efficient claims processing
  • Access to digital tools to help enhance patient care
