Person-centered planning is a collaborative process where people using behavioral health services participate in the development of service goals and in the selection of services provided. Person-centered planning is strengths-based and focuses on an individual’s abilities, preferences, and goals. Individuals using services and their significant others are core participants in the development of treatment goals and plans.
To further the utilization of this approach, Humana Healthy Horizons® in Louisiana and all managed care organizations (MCOs) providing coverage for Louisiana residents eligible for Medicaid offer training sessions on patient-centered planning.
During the training sessions, providers can ask questions. Although different MCOs offer the training sessions, the information provided is applicable to all MCOs. You do not have to be contracted with the hosting MCO to attend the training.
In 2023, Humana Healthy Horizons is hosting 2 of an expected 12 trainings on this topic. The schedule linked below includes information about the MCO hosting the training in each month as well as links for you to use to register for Humana-led trainings.
