Our eligible members can earn rewards for getting a COVID-19 vaccine

Humana Healthy Horizons in Florida members 12 and older can qualify to earn $20 in rewards after getting a COVID-19 vaccine. Our members who are 18 and older can qualify to earn $20 in rewards through Go365 for Humana Healthy Horizons™. Our members between the ages of 12 and 17 can earn a $20 gift card after we receive a claim for the vaccine from their provider.

For members between the ages of 12 and 17

All members between the ages of 12 and 17 can qualify to earn a $20 gift card after getting a COVID-19 vaccine. To qualify:

  • He or she must be fully vaccinated against COVID-19
  • We must have a claim on file from his or her provider

After we receive a claim, we will mail the member a $20 gift card to the address we have on file.

Members who already received a COVID-19 vaccine and have had Humana Healthy Horizons in Florida coverage since April 1, 2021, or before will be eligible for a $20 gift card as long as:

  • They are between the ages of 12 and 17
  • We have a claim on file from their provider for their COVID-19 vaccination(s)

For members 18 and older

All members who are 18 or older and enroll in Go365 for Humana Healthy Horizons can earn $20 in rewards after submitting proof of receiving a COVID-19 vaccine. To be eligible for this reward, our members must:

  • Register an account through Go365 for Humana Healthy Horizons™
  • Get a COVID-19 vaccine
    • If getting the Moderna vaccine, he or she must get both doses
    • If getting the Pfizer vaccine, he or she must get both doses
    • If getting the Johnson & Johnson vaccine, he or she must get one dose
  • Log in to their Go365 for Humana Healthy Horizons account
  • Upload an image of their completed vaccination card

Members who received a COVID-19 vaccination before we made this reward opportunity available still can earn $20 in rewards.

Members have 90 days from July 1, 2021, to upload an image of their completed vaccination card. To be eligible, members must have received their COVID-19 vaccination after April 1, 2021, and be:

  • 18 and older
  • A Humana Healthy Horizons in Florida member at the time he or she received the COVID-19 vaccine
