Patients who have been discharged from a hospital or skilled nursing facility (SNF) receive 10 frozen, packaged, low-sodium meals and can choose from regular, pork-free, fish-free, pureed, diabetic, vegetarian, gluten-free, kosher (in select areas) and renal-supportive options. Patients can receive meals in conjunction with up to four hospital/SNF admissions per year.
Patients who are eligible for the chronic condition meals program receive 20 frozen, packaged meals that support the special dietary needs of that chronic condition. Patients with multiple conditions can receive multiple benefits.
Patients may call 1-866-96MEALS (1-866-966-3257 or TTY: 711) to see if they are eligible for Well Dine under their plan benefits and to request participation in the program. Representatives are available Monday through Friday, 8 a.m. to 9 p.m. Eastern time.
A discharge nurse or a Humana At HomeSM care manager also may order meals for a patient. Along with their meals, patients receive a guide to healthy eating.
Meal delivery is arranged by Independent Living Systems (ILS). When a qualifying patient requests meals for either the discharge or chronic meal program, Humana and ILS send a fax to the patient's primary care provider or treating physician. Practitioners have a 24-hour window in which to change or cancel the meal request due to medical concerns. After that, ILS initiates meal delivery.
If a practitioner wants to change or cancel a meal request due to medical concerns after this time frame, he or she should call ILS at 1-866-96MEALS (1-866-966-3257 or TTY: 711) Monday through Friday between 8 a.m. and 9 p.m. Eastern time.
