Humana's Transplant Services team will guide them through the entire transplant process — beginning when they become a candidate for transplant all the way through one year after they received the transplant. Team members can help patients identify centers offering specific types of transplants and help them find lodging, transportation and other services available from those centers.
What is a transplant evaluation?
A transplant evaluation includes consultations, tests and X-rays ordered by the transplant program doctors for the patient and potential donors before transplantation.
How does a member receive a transplant evaluation referral, and who is responsible for coordinating referrals between the doctor and other specialists?

What type of help will the transplant care coordinators provide to a member?
The transplant care coordinator will help coordinate the member's transplant needs, explain benefits and help with any other transplant issues the member may have.
How long will the transplant care coordinator work with the member?
The care coordinator will work with the member from the initial evaluation up to one year after the transplant.
