Humana Medicare Rewards program flyer

Humana offers a rewards and incentive program to encourage eligible Medicare members to get needed preventive screenings and tests. Members who complete four eligible activities within a single calendar year will receive a $50 gift card.

The program is designed to reinforce engagement with preventive care activities and support your efforts to encourage your Humana-covered patients to get the screenings and tests they need.

Note that this program is for eligible Humana members only and currently is available only on limited plans (in Puerto Rico and on certain Group Medicare Advantage plans). Other Medicare Advantage plans may include access to the Go365 program. The Centers for Medicare & Medicaid Services (CMS) does not allow for the marketing or distribution of rewards and incentive materials to prospective members prior to enrollment.
