When submitting multiple-page corrected claims on paper, health care providers and their billing staffs need to include the words “page 1 of <insert total number of pages>,” “page 2 of <insert total number of pages>,” etc., on each page of the claim. Without the specification of total pages included, the following will result:

  • Each page will be considered a separate claim.
  • The member will receive multiple explanations of benefits (EOBs).
