According to the Centers for Disease Control and Prevention, chronic diseases lead to seven in 10 deaths each year, and treating people with chronic conditions accounts for more than 85 percent of the country’s health care costs. This trend in chronic conditions isn’t sustainable, and there’s no single cure.
That’s why we’re convening community and clinical leaders to solve this national problem at a local level. Together, we have a common mission: to make the communities we serve 20 percent healthier by 2020 by making it easy for people to achieve their best health.
In our Bold Goal 2016 Progress Report, we share our collective work from the past year in places like San Antonio, Tampa Bay, Louisville and Natchez, Mississippi.
We’re proving what’s possible when business and nonprofit agencies, religious organizations and government groups, academia and physician communities come together – all local and pulling in the same direction. We’re finding that’s how we create a movement that leads to better health.
