Earn Vitality Points for Vision & Dental Exams

Now, checking for healthy eyes and pearly whites are even more rewarding.

We’re excited to announce that HumanaVitality is making it even easier to earn Vitality Points when you and your family take steps to live healthier. For the first time, members will be able to earn Vitality Points for completing routine vision and dental exams.

Beginning January 1, HumanaVitality members on Humana and non-Humana vision and dental plans can earn Vitality Points for submitting proof of a routine exam. This is great news for the entire family because these exams will also be part of the Vitality Kids activities program. Dependents 18 years and younger can earn 100 Vitality Points for routine vision and dental exams.

Earning Vitality Points for vision and dental exams won’t always require a form--we’re working to make it automatic by spring of 2014!* Those are rewards you can see, even if you’re sporting some cool frames.

All HumanaVitality members must submit the Prevention Activity Form as proof of exam completion.

After Spring 2014:
For members with Humana Dental and Vision plans, Vitality Points will be earned automatically when dental and vision exam claims are processed*

Routine Vision Exam

Members, ages 18-64:
200 yearly maximum

Dependents, ages 18 and younger:
100 yearly maximum

Once per program year
