Role of a caregiver
As a caregiver, you have an important role. You may provide/perform:
- Companionship
- Health and wellness support
- Household chores
- Personal care
- Safety checks
You also may help someone:
- Find activities to do together
- Get in and out of the house safely
- Manage finances
- Set up a safe home
- Shop for groceries and cook
- Stay in touch with loved ones
Caregiver access
To let someone access your health information, you must sign and submit a Consent for Release of Protected Health Information (PHI).
This form will let caregivers view your plan information and get access to free support and resources from Humana. Caregivers also will get a free subscription to Humana’s quarterly caregiver newsletter.
You can give a caregiver access:

- With a paper form
