We know keeping everything organized can be a challenge when it comes to your healthcare. Part of your long-term care (LTC) plan is having the help of a care coach to assist you.

A care coach is your contact for getting services through Humana’s long-term care program, as well as health and wellness services and disease management.

Member Handbook

What does a care coach do?

We provide long-term support and services to help you live in your home and community. Our goal is to help you live where you want for as long as you want. Your care coach makes sure you and your caregiver know your options. If you want to live in an LTC facility, your care coach can give you information about:

  • Facilities in your area
  • How much living in an LTC facility will cost
  • The doctors you may be able to see

Your care coach also will:

  • Act as your supporter
  • Connect you to community services, in addition to your Humana benefits, to help improve your quality of life
  • Help you set and reach goals (e.g., personal goals, as well as your goals for transitioning from an LTC facility back to your home and community)
  • Review your care plan every 90 days and assess your needs based on your health and living situation
  • Visit you where you live (e.g., your home or LTC facility) and make sure the services you get meet your needs
  • Work with you, your caregiver, and your doctor to develop a care plan to help you meet your LTC needs
  • Work with your doctor to make sure you get behavioral healthcare, if needed

Care plan

You will work with your care coach, your caregiver, and your doctor to create your care plan. The plan will list:

  • The services you can get
  • How often you get the services

The plan is based on:

  • Your healthcare history
  • Current health needs
  • Living situation
  • The support you have from your family and friends

Your care plan may change as your needs change. Before changing your care plan, your care coach will talk with you and/or your caregiver and your doctor, if possible.

If you have questions about your care plan, ask your care coach.

Emergency plan

Your care coach will help you make an emergency plan. This plan gives details about how to prepare for a disaster or emergency.

The plan may include information like:

  • Creating personal and family plans for shelter
  • Dealing with special healthcare needs
  • Evacuation and emergency preparedness for people with disabilities and their caregivers
  • Local shelter listings
  • Special needs registries

Disease management

We designed our disease management program to help our members understand and manage any diseases they have and follow their doctors’ plans of care.

Members enrolled in our disease management program will get support from a nurse who will:

  • Provide education and support to help you learn how to control diseases such as Alzheimer’s/Dementia, Asthma, Cancer, Congestive Heart Failure, HIV/AIDS, Hypertension, and Sickle Cell Disease
  • Help you find resources in your community
  • Help you talk with your doctor
