Real-time Find a Doctor service
Our Find a Doctor service includes real-time information about doctors in our network. Search for doctors, specialists, and practices in your area.
To find doctors accepting new patients, who speak your language and offer the services you need:
Use our Find a Doctor service - Click on the Medical tab (stethoscope icon)
- Enter your ZIP code
- Select your lookup methods (e.g., coverage type or Member ID)
- Select LA Medicaid 2023 as your network
- Click select

To use our online Find a Doctor tool go to Humana.com/FindaDoctor.
Enter your ZIP code, select Medicaid as your coverage type, and select Network from the drop-down menu.
You can then search by the name of the doctor or facility, specialty, such as cardiology, or a condition such as ear infection.
You also can choose All and type any text into the search.
Then click Search.
Click Update my search to search for something else.
If you need help finding and choosing a doctor, call the number on the back of your enrollee ID card to speak with an Enrollee Services representative.

We update the below Provider Directories at the start of every month. You can view in English or in Spanish information about doctors in our network.
Region 1 - Metropolitan: Jefferson, Orleans, Plaquemines, and St. Bernard parishes

To change your primary care provider (PCP):
Use our online PCP change request service in your MyHumana account service (desktop version only)- Call us at 1-800-449-3810 (TTY: 711), Monday – Friday, 7 a.m. – 7 p.m.
Find a dentist
Dental benefits for members 21 and older
Humana Healthy Horizons® in Louisiana members who are 21 and older get dental services through DentaQuest.
To find a dental provider:
Use the online DentaQuest directory (find a dentist)- Select Humana of LA Adult Medicaid VAB
- Click Confirm selection
- Call 1-800-685-0143 (TTY: 1-800-466-7566), Monday – Friday, 7 a.m. to 7 p.m.
Dental benefits for members younger than 21
Humana Healthy Horizons in Louisiana members who are under 21 can get dental services from either DentaQuest or MCNA Dental. You must choose one of these providers for your dental services.
To find a DentaQuest dental provider:
Use the online DentaQuest directory (find a dentist)- Select LA Medicaid EPSDT Dental Program (under age 21)
- Click confirm selection
- Call 1-800-685-0143 (TTY: 1-800-466-7566), Monday – Friday, 7 a.m. to 7 p.m.
To find an MCNA dental provider:
Use the online MCNA Dental directory (online provider directory)- Enter your ZIP code
- Select Louisiana EPSDT Dental Program
- Call 1-855-702-6262 (TTY: 1-800-955-8771), Monday – Friday, 7 a.m. to 7 p.m.
