The website is organized into four sections:

- Metrics & Management
- Social Determinants
To navigate through the site, you can begin by taking the Readiness assessment to identify your unique areas of opportunity along the value-based continuum. From there, you can navigate to the Collaboration section to learn about guiding principles for successful partnerships between provider groups and health plans. You can also navigate to the Metrics & Management section to identify key performance indicators and best practices that can positively impact overall practice performance. The Social Determinants section will take you to a separate Humana website focused on the impacts social determinants of health can have on the total health outcomes of patients.
To request access, click on Register/Login from the top navigation bar and then click Register now. You will be taken to a registration form – fill out the requested information.
