Acupuncture for chronic lower back pain Up to 20 visits per year under the following circumstances:

For the purpose of this benefit, chronic low back pain is defined as:

  • Lasting 12 weeks or longer;
  • Nonspecific, in that it has no identifiable systemic cause (i.e. not associated with metastatic, inflammatory, infectious, etc. disease);
  • Not associated with surgery; and
  • Not associated with pregnancy

Your plan allows services to be received by a provider licensed to perform acupuncture or by providers meeting the Original Medicare provider requirements.

Over-the-counter (OTC) allowance Up to $65 per quarter allowance enables members to purchase products that support common occurring conditions such as:

  • Pain relivers
  • Cough and cold relief medicine
  • First aid equipment that do not require prescriptions.

Unused amounts do not roll over to the next quarter.
