Topical fluoride treatment and sealants for children 16 and younger
Periodontal maintenance
Space maintainers for children 15 and under
Oral cancer screenings for people age 40+

Basic services

Emergency care for pain relief
Simple extractions
Fillings
Oral surgery
Prefabricated stainless steel crowns

People with this plan may receive a discount for additional basic services, such as crowns and harmful habit appliances for children.

With Preventive Plus, your employees and their covered family members will pay nothing out of pocket for preventive care and a percentage of the most commonly used basic services.
