What is telephonic care management?
Humana’s telephonic care management is a service that works with you and your healthcare providers to help you achieve your best health at home. Eligible members with a Medicare Advantage plan can get a personal care manager to help you and your care team and provide the tools and confidence to better manage your health. All members with a Medicare Advantage Special Needs Plan (SNP) are eligible for telephonic care management at no additional cost.

Personal care manager
Healthcare professional. Coach. Friend. Your personal care manager can be there for you in many ways and be the person who:
- Works with your healthcare providers to support their plan of care
- Gets to know you as a person, including your likes, dislikes and wishes
- Partners with you, serving as your primary point of contact at Humana
- Helps you understand your Medicare and Medicaid benefits (if eligible for Medicaid)
Care team
Your care team works together to guide you on your health goals and help you achieve your best life. Members of your care team can vary depending on your needs, but may include:
- Healthcare providers
- Behavioral health specialists
- Social worker
- Pharmacist
- Dietician
- Care coach
- Caregiver
Healthcare support throughout the year
Keeping track of appointments and medications can be a hassle. Telephonic care management offers everyday support in many areas, such as:
- Appointment scheduling
- Medication management
- Health education and coaching
Your care team will be available by telephone for many services. You can call them with any questions you have. Calls can be scheduled for medical reviews, primary care physician (PCP) outreach and to connect with community resources.
Transition of care support
The transition from a hospital or medical facility to home can be challenging. But for eligible Medicare Advantage (MA) plan members, your care management team can help you every step of the way.
Here’s how it works: When you’re discharged from a hospital or medical facility, a Humana care manager, backed by a team of healthcare professionals, will be available to work closely with you, your provider and/or caregiver. The goal is to reduce the chance of a hospital readmission by helping you:
- Visit your provider or specialist within 7 days of your discharge
- Understand your medications and possible side effects
- Obtain any necessary medical equipment
- Keep your appointments with home health if you were set up for these visits at time of discharge
Your Humana care manager can also help you maximize benefits that may be included in your plan, like meal delivery. This way, you can focus on getting better while we work to try and help improve your overall quality of care.
