Humana works with the Washington State Healthcare Authority (HCA) and local Care Coordination teams to provide Health Home services to our Medicare/Medicaid, Dual Eligible membership. You are eligible for this program if you’re a Health Home engaged member under Medicaid and want to join 1 of Humana’s Washington DSNP plans in 2023.*
* If you are not already an engaged Health Home member when you join our plan, you will not be eligible for the program.
