Humana works with the Washington State Health Care Authority (HCA) and local Care Coordination Organizations to provide the Health Home program to our Medicare/Medicaid, Dual Eligible membership. You’ re eligible for this program if you have at least one chronic condition and are at risk for another, have a PRISM* predictive risk score of 1.5, and meet Medicaid eligibility criteria. You’re also eligible for this program if you’re a Health Home engaged member under Medicaid and want to join one of Humana’s Washington DSNP plans.
