Loyalty Plus dental plan

This plan offers day-one coverage with no waiting periods, and rewards you for your loyalty. You can see any dentist you want and your benefits increase over time. The percentage the plan pays toward services increases each year, up to the 3rd year. In the 1st year, the plan allows a maximum of $1,000 per individual, but that limit rises to $1,500 per person by year 3.

Major dental services coverage

Includes coverage for root canals, crowns, and dentures, available up to the annual benefit maximum.

• 20% covered for the 1^st year
• 30% covered for the 2^nd year
• 50% covered for the 3^rd year and on
