Day 1 coverage with no waiting period is a great start. But the longer you stay in our Loyalty Plus plan, the more your annual plan maximum will grow. It starts at $1,000 the first year and increases to $1,500 by the third year. Also, the percentage paid for basic and major services increases over the first 3 years.

Dental PPO plans are not offered in all states.

Dental and vision plans, excluding Dental Savings Plus, may have a minimum one-year initial contract period.

Payment may include an administration fee. Association membership and fees may be required on some plans in some states. A one-time, non-refundable enrollment fee may apply (the fee is non-refundable as allowed by state requirements). Applicable fees are disclosed at time of enrollment.

For Colorado: The Network Access Plan, which describes an access plan specific to your network, is available by calling the customer service number found on your Humana Vision ID Card/Dental ID card and requesting a copy.
