Recent healthcare events

Notice of Humana misdirected provider documents

Background

Humana is providing notice of an event that involves protected health information. Currently, there is no indication that information has been misused because of this event. We are providing information about the event, our response to it, and steps our members can take to protect themselves, should individuals feel it appropriate to do so.

What Happened?

On December 8, 2023, Humana became aware that Explanation of Payment documents intended for providers were sent to the incorrect address due to a system programming error.

What Information Was Involved?

The information included on the Explanation of Payment documents included: First and last name, Humana ID number, Provider name, Dates of Service, Claim Payment Information.

What is Humana Doing?

Humana has policies in place to maintain the privacy of our members’ information. Humana immediately identified the cause of the error and corrected it. Through testing we confirmed the system is now operating correctly.

What individuals can do

We are not aware of any misuse of this information. We encourage our members to contact Humana’s Privacy Office immediately if they or find unfamiliar activity on the statements that they receive from Humana.

Notice of Humana document mailing error

Background

Humana is providing notice of an event that involves protected health information. We are providing information about the event, our response to it, and steps our members can take to protect themselves, should individuals feel it appropriate to do so.

What Happened?

On December 14, 2023, Humana became aware that alternate format (large print/braille) health plan communications were mailed to the incorrect Humana member. It was discovered that while fixing an unrelated issue with coding, a new error was created that added a date/time stamp to the naming convention, which was not a unique identifier. This error caused the system to begin overwriting files as duplicates, resulting in one member receiving another member’s letter.

What Information Was Involved?

The information within these health plan documents included: First and last name, Address, Humana ID number, Provider name, Dates of Service, Claim Payment Information, Prescription Medication Information, and Copay and Premium Information.

What is Humana Doing?

Humana has policies in place to maintain the privacy of our members’ information. Humana immediately identified the cause of the error and corrected it. Through testing we confirmed the system is now operating correctly.

What Happened?

On January 12, 2024, Humana’s vendor, Broadridge Output Solutions, Inc. (Broadridge) experienced a breakdown between printers. The breakdown caused Explanation of Benefits (EOB) information to print on the back of another member’s EOB statement.

What Information Was Involved?

The information within these health plan documents included: Name, Claim information, Provider name, Gender, Copay information, Deductible and coinsurance information.

What is Humana Doing?

Humana has policies and procedures in place to maintain the security of information and we are taking additional steps. On January 24, 2024, Broadridge switched Humana’s print output to different printers to prevent a similar error in the future. Also, post-printing quality checks will be performed to ensure front-to-back printing is correct before the document is placed in the envelope.

What Individuals Can Do

We are not aware of any misuse of this information. We encourage our members to contact Humana’s Privacy Office immediately if they find unfamiliar activity on the statements that they receive from Humana.
