Compassionate care in the last phases of life

Hospice care is a team approach provided by professionals and volunteers that focus on comfort measures for those in the last phase of life. The hospice team is accessible around the clock for support and visits as appropriate. Support continues for family and friends with grief and bereavement counseling.

Hospice care provides physical, emotional and spiritual support for patients with a terminal illness. Your care team will design a plan with you and your family to provide comfort and make the most of the remaining months, weeks and days.

Eligibility for hospice care

Hospice care is for people with a life-limiting progressive illness with a prognosis of 6 months or less who have chosen comfort care over treatment to cure the illness. Electing hospice care early in the prognosis allows the patient and family to receive the full benefit of hospice care.

Some diagnoses commonly eligible for hospice care include:

  • Heart disease
  • Respiratory diseases
  • AIDS/HIV
  • Alzheimer’s/Dementia
  • Liver/Renal Failure
  • Neurologic diseases
  • Cancer
  • Stroke or coma

What to expect from hospice care

Your hospice care team will come to you in your place of residence and create a care plan based on your wishes and goals. The care plan is then carried out by the hospice team, which may include but is not limited to:

  • Experienced nurses
  • Chaplain or bereavement counselor
  • Patient’s physician
  • Social worker
  • Medical director
  • Hospice aides

Hospice services are focused on the management of pain and other symptoms. Emotional or spiritual support are provided based on the needs of the patient.

When you or a loved one are ready to discuss hospice care, talk to your healthcare provider. A healthcare provider can refer you to hospice care when you are clinically and emotionally ready.

All members on Humana Medicare Advantage plans are eligible to receive hospice care at little to no out-of-pocket costs. Your hospice team can advise you of the full benefits available to you under hospice care when you choose to elect hospice care.
