Hospice care

Hospice care at home provides physical, emotional and spiritual support for patients with a terminal illness. Your care team will design a plan with you and your family to help you live each day as fully and comfortably as possible. Hospice care:

  • Requires a terminal diagnosis
  • Begins after treatment has stopped
  • Aims to provide relief of pain and other symptoms of the terminal illness
