Our Preventive Plus plan covers commonly used preventive and basic services, including exams, X-rays, cleanings and fillings.

- 3 routine cleanings per plan year
- Exams and X-rays
- Topical fluoride treatment and sealants for children 16 and younger
- Periodontal maintenance
- Space maintainers for children 15 and under
- Oral cancer screenings for people age 40+
Basic services
- Emergency care for pain relief
- Simple extractions
- Fillings
- Oral surgery
- Prefabricated stainless steel crowns
People with this plan may receive a discount for additional basic services, such as crowns and harmful habit appliances for children.
With Preventive Plus, your employees and their covered family members will pay nothing out of pocket for preventive care and a percentage of the most commonly used basic services.
