Efficiency is a low-cost option for employees to get the preventive care they typically need, like annual checkups.
Here’s an example of how an Efficiency plan works:
- Individual deductible (medical plus pharmacy) – $5,500–$8,150*
- In-network coinsurance – 50%–100%*
- Individual maximum out-of-pocket – $8,150*
- Preventive exams, screenings and immunizations – 100%
Once the deductible is met, the plan pays the coinsurance percentage of covered services and prescriptions until the out of pocket maximum is reached. After that, the plan pays 100% of all in-network covered services.
*All costs are for in-network providers. Plan option costs vary depending on the plan chosen. Family deductible and out-of-pocket maximum amounts are twice the individual amounts.
