This low-cost solution is made for those whose eye exams are covered by their medical plan, providing coverage for frames, eyeglass lenses, and contact lenses through Humana’s nationwide vision network.

If your employees already have coverage for vision exams through another plan, but not for their materials, our Materials Only plan offers a low-cost solution to get them what they need.

- Low-cost premiums to cover frames, lenses, and contacts
- Lens options like scratch-resistance and anti-reflective coating from network providers
- Discounts on non-covered items – May be available up to a 20% discount on items not covered by the plan when received from network providers
Our Materials Only plan comes in 2 plan types:
Humana Vision Materials 130*- Up to a $130 allowance for frames, and members may receive 20% off the remaining balance
- Up to a $130 allowance for conventional contact lenses, and members may receive 15% off the remaining balance
- Up to a $130 allowance for disposable contact lenses
- $15 copayment for single, bifocal, and trifocal lenses
- Up to a $160 allowance for frames, and members may receive 20% off the remaining balance
- Up to a $160 allowance for conventional contact lenses, and members may receive 15% off the remaining balance
- Up to a $160 allowance for disposable contact lenses
- $10 copayment for single, bifocal, and trifocal lenses
