Discounts on non-covered items – May be available up to a 20% discount on items not covered by the plan when received from network providers

Our Materials Only plan comes in 2 plan types:

Humana Vision Materials 130*

Up to a $130 allowance for frames, and members may receive 20% off the remaining balance
Up to a $130 allowance for conventional contact lenses, and members may receive 15% off the remaining balance
Up to a $130 allowance for disposable contact lenses
$15 copayment for single, bifocal, and trifocal lenses

Humana Vision Materials 160*

Up to a $160 allowance for frames, and members may receive 20% off the remaining balance
Up to a $160 allowance for conventional contact lenses, and members may receive 15% off the remaining balance
Up to a $160 allowance for disposable contact lenses
$10 copayment for single, bifocal, and trifocal lenses
