That’s just 1 reason why completing the HRA is worth the 5 minutes it takes. It’s a simple way for us to help make sure you’re using all the benefits you’re entitled to.

The most rewarding thing about being a care management support assistant is that I get the opportunity every day to assist people navigating through the challenges of life today and getting them the support they need through our team.

I have an awesome team. I mean, my team - literally I feel like family. I'm a nurse care manager. I'm here to collaborate with your doctor.

The health risk assessment is a useful tool to give us an understanding as to our members’ needs. I just need to go through some simple questions. We work through that with our members. The assessment’s completed then you explain to the member how the process works. So the rn care manager would reach out to the member, typically within the next 2 to 4 weeks time. So the support assistant helps me complete the health risk assessment. They are non-clinical. They ask all the questions in the assessment and then me, as a clinical nurse, I follow up so we  coordinate to get the assessment completed together. The members, when they complete the assessment, they often themselves identify things perhaps they've never even really spoken to someone about. And through that building of rapport and conversation, we're able to discuss those needs and move them on to the support they much need. I can help you complete the HRA if you haven't done one in the past year. It only takes a few minutes. I am going to ask you some personal questions if that's okay, so let's get going. Let's see how I can help you and support you. By completing the health risk assessment, once you've done that you will have access to a team of care managers - our RN care managers as well as our social services team. So you will be able to get assistance maybe for health related concerns or social support needs. Just take a few minutes to complete the HRA so we can give you support and guidance. How the Health Risk Assessment helps us help you

The Medicare Health Risk Assessment, or HRA, is a fancy name for a simple health questionnaire. We use this short survey to learn more about you and any challenges you may be facing. Then we can look at the Humana plan you’ve chosen to make sure you’re not missing out on any plan benefits that might help make your life a little easier. The assessment takes about 5 minutes and asks simple questions about:

Health history

Medical conditions, surgeries, medications, allergies and family health history

Lifestyle and habits

Diet, exercise routine, smoking, alcohol consumption and substance abuse

Mental health

Stress, depression, anxiety and overall emotional well-being

Functional challenges

Assessments of daily living activity, physical mobility and ability to live alone

Social challenges

Family, friends or other support, or access to non-healthcare related resources you may need

Preventive services

Vaccinations, screenings and preventive care

Once your assessment is reviewed, 1 of our care managers will reach out to talk with you about any challenges you may be facing. The goal? To match your plan’s benefits to your personal healthcare needs.
