Understanding MLTSS and how it supports you

Managed Long-Term Services and Supports, or MLTSS, is an Illinois Medicaid program for eligible members who need long-term care or help with daily needs. MLTSS may support care at home, in the community, in a supportive living setting or in a nursing facility.

Humana Healthy Horizons in Illinois MLTSS helps coordinate covered long-term services and supports, certain behavioral health and substance use services, transportation to covered appointments and care coordination.

Who can enroll?

You may qualify if you are eligible for the IL Fully Integrated Dual Eligible (FIDE) plan and need long-term services and support. This may include people who live in a nursing facility, receive supportive living services, or qualify for home- and community-based services (HCBS) waiver services.

MLTSS may be for members who:

  • Are eligible for Medicare and Medicaid
  • Are 21 years or older
  • Live in Illinois
  • Need long-term care or help with daily needs

Some members may be required to enroll in an MLTSS plan if they meet state program rules and are not exempt.
