Your plan may include an over-the-counter (OTC) monthly credit. Please call a Customer Care specialist to verify your eligibility, determine your benefit amount, or request an OTC order form at 1-800-457-4708. If you use a TTY, call 711, 7 days a week, 8 a.m. – 8 p.m.
