The Landmark team doesn’t replace your primary care doctor or other specialists. Continue to see the providers you know and trust, while getting additional support from Landmark’s in-home medical team of doctors, nurse practitioners, pharmacists and more.

Landmark providers are available any time of the day or night so you can get medical advice even when your regular doctor’s office is closed.

Find out if you’re eligible

As a Humana Medicare Advantage member, you may be eligible for Landmark’s care management program if you have a combination of any of the following chronic conditions:

  • Atrial fibrillation
  • Cancer
  • Cerebrovascular disease
  • Chronic kidney disease
  • Coronary heart disease
  • Diabetes
  • Disabling conditions (e.g., paralysis, amputation)
  • End-stage renal disease
  • Fluid and electrolyte disorders
  • Heart failure
  • Pressure ulcers with necrosis: stage 4
  • Protein-calorie malnutrition
  • Pulmonary disease
  • Severe chronic liver disease
  • Vascular disease
