At Humana Healthy Horizons we work with you and provide you with the care and support you need as a PathWays member – so you have more time to focus on what you love. Enrollment began February 2024. Call 87-PATHWAY-4 (877-284-9294) to learn more about this program.

Humana Healthy Horizons in Indiana is proud to partner with Indiana Family and Social Services Administration (FSSA) to transition to a new model through the Indiana PathWays for Aging program. The following Indiana PathWays for Aging populations may be eligible to pick a health plan:

Individuals that are 60 years and older, blind or disabled, and have limited income and resources

Humana Healthy Horizons - Indiana PathWays for Aging difference

As a PathWays member with Humana Healthy Horizons in Indiana, you’ll get the same healthcare services you get today, such as:

Dental Services, including cleanings and x-rays
Emergency services
Hospice Care
Nursing Facility services
Transportation services, including emergency and non-emergency transportation
Vision Care services, including vision exams and eyeglasses

FSSA will send you a letter with information about choosing a health plan. As the date approaches, we’ll share information on this website about our plan and its coverage, which will include coverage for behavioral health, hearing, medical, pharmacy, transportation, and vision services.
