Humana’s Institutional Special Needs Plan

Humana now offers an Institutional Special Needs Plan (I-SNP) in certain locations. I-SNP is a type of Medicare Advantage plan that combines all the benefits of Original Medicare (Parts A and B) with prescription drug coverage (Part D) and is designed for residents in a long-term care facility.

Our on-site nurse practitioner (NP) is dedicated to supporting our members’ complex care by partnering with your primary care physician and long-term care facility to provide enhanced, coordinated care services.

This plan was created to build upon Original Medicare and for members who reside in a long-term care facility

Humana’s I-SNP is designed to enhance coordinated care and includes specialized benefits that offer:

  • On-site dedicated NP for comprehensive care management services
  • Proactive and follow-up care to achieve your care goals
  • Comprehensive medication reviews
  • Coordinated care with your primary care physician (PCP) and facility care team
  • Improved education and communication around care transitions and care plans with you and your loved ones

In addition to covering your Medicare and prescription drugs, Humana Together in Health provides these benefits to you at no additional cost:

  • Primary care and behavioral health virtual visits
  • On-site & dedicated NP
  • Full preventative coverage and comprehensive dental allowance
  • Over-the-counter allowance to cover your health and wellness products
  • Vision & coverage toward hearing aids
  • Transportation, and much more

You can enroll or disenroll year-round in Humana’s I-SNP. There is no need to wait for the Annual Enrollment Period.

You are eligible for Humana’s I-SNP if you:

  • Are a Medicare beneficiary eligible for Medicare Part A
  • Are enrolled in Medicare Part B
  • Are a resident of a participating long-term care facility
