Person-centered planning is a collaborative process where people using behavioral health services participate in the development of service goals and in the selection of services provided. Person-centered planning is strength-based and focuses on an individual’s abilities, preferences, and goals.
To further the utilization of this approach, Humana Healthy Horizons and all MCOs providing coverage for Louisiana residents eligible for Medicaid offer training sessions on patient-centered planning.
During the training sessions, providers can ask questions. Although different MCOs offer the training sessions, the information provided is applicable to all MCOs. You do not have to be contracted with the hosting MCO to attend the training.
