Sometimes, you need help getting around. This is why Humana Healthy Horizons® in Illinois members have coverage for medical transportation. We want to make it easy to get the care you need.

We’ll help you get a ride
We will help you get to your doctor visits and the pharmacy. It’s easy and free to plan a ride.
- You can get a ride to your doctor, the pharmacy, or a health care visit.
- You can get as many rides as you need.
- You can plan a one time ride, or you can plan rides for regular doctor visits or other appointments.
- Your ride type will be based on your needs. It may be:
- An independent or volunteer driver picking you up
- Mileage reimbursement for a friend or family member
- A ticket for public transit
- A taxi

How to plan a ride
Contact Medicaid Transportation Management (MTM) at 855-253-6867 (TTY: 711)
Monday – Friday 8 a.m. – 8 p.m., Central time.
Emergency transportation
If you have a medical emergency, dial 911. Humana Healthy Horizons in Illinois members are covered in case of emergent and urgent medical needs. You will receive an ambulance ride to the nearest emergency room.
