We ask all members to complete an HRSA. The HRSA is your chance to: 

• Tell us about your health and well-being
• Tell us about your health goals
• Share information about your health related needs
• Get paired with a care manager who can help you reach your health and well-being goals

Completing your HRSA is important for our pregnant members, especially during the first 60 days of enrollment in Humana Healthy Horizons in Illinois. To complete your HRSA : 

• submit your HRSA through Go365 for Humana Healthy Horizons.
• Call us at 800-787-3311 (TTY: 711), Monday – Friday, 8 a.m. – 8 p.m., Central time, OR
• Register for a MyHumana account opens in new window and submit the HRSA online

During and after pregnancy, you and your body will go through many changes. Most of those changes will be normal. Some changes may not be normal. They could be a sign of serious problems that can cause long-term effects or even death. 

You may not know how to tell the difference between normal and abnormal changes. But, you know when something feels wrong. If you are worried about symptoms or changes, talk to your doctor. We want you to make sure you get the care you need.
