Helping dual-eligible members eat healthier

If you’re eligible for (or enrolled in) Humana’s Medicare Advantage Dual Eligible Special Needs Plan (D-SNP), your plan may include the Humana Healthy Foods Card benefit. The benefit may help Humana members who are short on money for food. Qualified members can use the card at participating stores.

This card gives you a monthly allowance to spend on approved healthy foods at participating stores across the U.S. Qualifying items include milk, eggs, bread, juices, water, yogurts, fruits, vegetables and more. Best of all, this allowance reloads every month!

How to use the Humana Healthy Foods Card

  1. Activate your card
  2. Shop at a participating store
  3. Pick out any approved healthy foods you want
  4. Show your Humana Healthy Foods Card to the cashier during checkout and have it scanned
  5. Your card will cover the costs of approved foods in your cart up to the approved limit

What is a D-SNP?

Dual-SNPs are for individuals who are entitled to Medicare and who are also eligible for assistance from a state Medicaid program. SNPs are available to anyone who meets the specific eligibility requirements of the plan, which can vary by state and service area.
