Chronic Condition Care Assistance Exclusions List

Humana offers the Chronic Condition Care Assistance benefit to members diagnosed with certain chronic conditions and meet the definition of chronically ill. This special supplemental benefit helps cover the costs of approved health-related and non-health related items and services like clothing, meals, transportation and hygiene products.

Speak with your care manager to determine if the items and services covered by the benefit will help you meet your unique healthcare goals. The items and services covered by the benefit are subject to Plan and CMS approval. 

Here is a list of exclusions to be aware of.

List of exclusions for 2025

Payments or procurement of any of the following through Chronic Condition Care Assistance is prohibited:

  • Application fees
  • Appliances
  • Broad membership-type programs inclusive of multiple unrelated services and discounts ( i.e. Amazon Prime, Costco, Sams, etc.)
  • Cost Share for Part D benefits (prescription benefits)
  • Cannabis products, paraphernalia, or services related to the procurement of
  • Deposits of any kind
  • Direct Member Reimbursement
  • DME purchased outright
  • Electronics (i.e. televisions, cameras, security systems, computers, telephones)
  • Entertainment (i.e cable, satellite, streaming services, green fees, event tickets)
  • Exercise equipment
  • Furniture items
  • Funeral planning and expenses
  • Gift Cards – any
  • Homeowner Association/ rental maintenance fees
  • Home Repairs or general maintenance
  • Insurance premiums – any
  • Moving fees
  • Payments to family members or live-in partners
  • Payments to collection agencies
  • Payments on accounts that are not the members or would benefit solely an individual that is not the member.*
  • Personal loans / credit card payments
  • Personal vehicles – anything related to
  • Pet Care or items beyond the payment of boarding during emergent situations. Providers that are not licensed or certified to provide the services requested.
  • Procedures that are solely cosmetic in nature and do not extend upon Traditional Medicare coverage
  • Service or Maintenance Agreements
  • Services received outside of the effective dates of the plan.
  • Tax liabilities – any
  • Warranties
  • Zero Balance Accounts – including prepayments of goods or services to be received at a future point in time.

*Payments may be made on accounts that are not in the member’s name only when the service address matches the service address that Humana has on file as the members permanent address.

Note: The exclusions list above is not exhaustive. There may be items we are unable to assist with that are not explicitly listed as a non-covered service. This list is subject to change based on plan or CMS discretion.
