Your health plan includes a care manager, help finding a doctor, and support for caregivers.

Understanding your Long-Term Services and Supports (LTSS)

Humana provides long-term support and services to help you continue living in your home and your community. Our goal is to help you live where you want for as long as you want.

Humana Healthy Horizons members get to work with a care coach who will:

  • Contact your doctor(s) so you can begin your services
  • Help you choose the doctor(s) to see
  • Work with you, your family, your caregivers, your doctor(s), and other healthcare providers to make sure that your services work with your medical care and other parts of your life
  • Work with you and your support system to develop a Person-Centered Plan of Care

If you need Long Term Services and Supports, a care coach will call you to discuss your needs.

Caregiver support

Caring for a loved one can be challenging. Humana offers support and resources to help you. Looking for the forms you need to grant a caregiver access? You can find those here, too.

Role of a caregiver

As a caregiver, you have an important role. You may provide/perform:

  • Companionship
  • Health and wellness support
  • Household chores
  • Personal care
  • Safety checks

You also may help someone:

  • Find activities to do together
  • Get in and out of the house safely
  • Manage finances
  • Set up a safe home
  • Shop for groceries and cook
  • Stay in touch with loved ones

Caregiver access

To let someone access your health information, you must sign and submit a Consent for Release of Protected Health Information (PHI).

This form will let caregivers view your plan information and get access to free support and resources from Humana. Caregivers also will get a free subscription to Humana’s quarterly caregiver newsletter.

You can give a caregiver access:

Download the necessary consent forms from the contacts and forms tab. Then, work with your loved one to complete them. Your Care Coach will need you or your authorized representative to sign the forms at your face-to-face visits.

211 program

The 211 program is a free, private service to help find local resources, including:

  • Food and nutrition programs
  • Shelter and housing resources
  • Utilities support
  • Disaster relief
  • Employment
  • Addiction prevention and rehabilitation programs
  • Reentry support for ex-offenders
  • Support groups
  • Safe help out an abusive situation

Caregiver support groups and communities

Most people need emotional support during their time as caregivers. Support can help set boundaries, improve communication, and boost coping skills.
