Non-emergency medical transportation
Your PathWays plan with Humana Healthy Horizons in Indiana includes ride benefits. In partnership with LCP Transportation, Humana provides rides to:
- Doctor visits and healthcare appointments
- Eligibility redetermination appointments with the state
- Urgent and repeating appointments
Available transportation will depend on your location and may include:
- A car or van
- A taxi or rideshare service
- A vehicle with a wheelchair lift
There is no additional cost for this service

How to plan a ride
- Contact LCP Transportation 800-936-2794 TTD/TTY available 24/7, 365
- Schedule your ride at least 48 hours in advance
- If you need to cancel, do so at least 24 hours before your appointment
Emergency Transportation
If you have a medical emergency, dial 911. Indiana PathWays for Aging members are covered in case of emergent and urgent medical needs. You will receive an ambulance ride to the nearest emergency room.
