Do you have both Medicare and Medicaid? A Medicare Advantage Dual Care Special Needs Plan (D-SNP) may be right for you.
Humana PathWays Dual Care Plan
Recently, Humana mailed some disenrollment letters on accident to Medicare Advantage members. Humana called any members impacted by this mistake and sent an apology letter. We apologize for this error and any confusion it caused. If you have any question about your eligibility status, please feel free to call the number on the back of your card.
How a D-SNP works
Medicare Advantage D-SNPs offer special benefits, including prescription drug coverage. Many of our D-SNPs include a Healthy Options Allowance
Who can enroll
You must be eligible for both Medicare and Medicaid to enroll. Plans vary by location, so you must live in a state where we offer D-SNPs. Humana coordinates your benefits to help you get the most out of your coverage.
If you live in the state of Indiana and have both Medicare and Medicaid, explore our Humana PathWays Dual Care (HMO-POS D-SNP)
Select plans offer $O Rx copay and Healthy Options Allowance rollover
Many Humana D-SNPs offer $0 copays for hundreds of covered prescriptions. There’s no deductible and no copay for all covered Part D prescriptions—even name brand ones.
Also, a Healthy Options Allowance rollover is included in some D-SNPs. While enrolled in the plan, any unused balance from 1 month will rollover to the next month and expire at the end of the plan year.
Let us help you choose a plan
In just a few simple steps, we can help you narrow down your choices based on what’s most important to you.
*The Humana Healthy Options Allowance is a special program for members with specific health conditions. Qualifying conditions include diabetes mellitus, cardiovascular disorders, chronic and disabling mental health conditions, chronic lung disorders, or chronic heart failure, among others. Some plans require at least two conditions and other requirements apply. See the plan’s Evidence of Coverage for details. If you use this program for rent or utilities, Housing and Urban Development (HUD) requires it to be reported as income if you seek assistance. Contact your local HUD office if you have questions.
**Dual Special Needs Plans have a $0 premium for members with Extra Help from Medicare (Low Income Subsidy). Members who don’t qualify for Extra Help may have a premium of up to $41.50 a month in select markets
